In consideration of volunteering to participate in any way in the activities listed on this website, I, for myself for personal representatives, assigns, heirs, and next of kin:
Acknowledge, agree, and represent that I understand the nature of the sports and activities in which I choose to participate and
that I am qualified, in good health, and in proper physical condition to participate in such activity. I further agree and warrant that if at any time I believe conditions to be unsafe, I will immediately discontinue further participation in the activity.

I understand that I must provide my own health insurance to cover injuries that may be sustained before, during, or after any participation in an activity advertised on this website.  Any person who I bring with me, of any age, must have active health insurance as well to cover any injuries that may be sustained before, during, or after any participation in or attendance at activities.
I fully understand that:
(a) Sports activities involve risks and dangers of serious bodily injury, including permanent disability, paralysis and death (“Risks”).
(b) These Risks and dangers may be caused by my own actions or inaction’s, the actions or inaction’s of others participating in the Activity, the condition in which the Activity takes place, or the negligence of the releasees named below including but not limited to slips, falls, collisions, equipment failure, bad decision-making, inattentive players and from equipment that has become worn, loose or damaged.
(c) There may be other risk, social and economic losses either not known to me or not readily foreseeable at this time; and I fully accept and assume all such risks and all responsibility for losses, costs and damages that I incur as a result of my participation or that of the minor in the Activity.

I Hereby release, discharge, and covenant not to sue ExecutiveSportsNetwork.com, their respective administrators, directors, agents, officers, members, volunteers, and employees, other participants, any sponsors, advertisers, and, if applicable, owner and lessors of premises on which the Activity takes place, (each considered one of the "releases" herein) from all liability, claims, demands , losses, or damages on my account caused or alleged to be caused in whole or in part by the negligence of the “releasees” or otherwise, including negligent rescue operations. This release applies to and binds my personal representative, heirs, and my family. If a member of my family under age (18), or guest accompanies me to an activity, I make this release and these representations on their behalf as well as my own, and I agree and assume responsibility for their safety.

I hereby grant the Executive Sports Network and Revolutionary Performance Management, Inc. permission to use my photograph in any and all publications for advertising and display purposes, including web site entries, print advertising, brochures, posters, postcards, banners, or in any other media, without payment or any other consideration in perpetuity.

I hereby authorize the Executive Sports Network and Revolutionary Performance Management, Inc. to edit, alter, copy, exhibit, publish or distribute this photo. I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my photo appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photograph.

I hereby hold harmless and release and forever discharge the Executive Sports Network and Revolutionary Performance Management, Inc. from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf of on behalf of my estate which may have or may have by reason of this authorization.

I am 18 years of age or older and am competent to contract in my own name. I have read this release, and I fully understand the contents, meaning, and impact of this release.

I HAVE READ THIS AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT AND HAVE SIGNED IT FREELY AND WITHOUT INDUCEMENT OR ASSURANCE OF ANY NATURE AND INTEND IT TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW AND AGREE THAT IF ANY PORTION OF THIS AGREEMENT IS HELD TO BE INVALID THE BALANCE, SHALL CONTINUE IN FULL FORCE AND EFFECT.
